Privacy Policy

This policy explains what information the Premier Spine Care website (premierspinecare.com) collects, how we use it, and the choices you have. Premier Spine Care is the spine surgery practice of Adrian P. Jackson, MD, with offices in Overland Park and Lenexa, Kansas.

Health information you give us as a patient, whether on this website, by phone or at our office, is also protected by the federal HIPAA privacy rules. Our Notice of Privacy Practices describes how we use and share it and your rights over it. You can get a copy at our office or by calling us.

Information you send us through our forms

  • New patient forms. When you complete our patient forms online, your answers are turned into a PDF in your own browser and sent to our office. They can include your contact details, insurance or claim details, and health history.
  • Referrals. When an adjuster, case manager, attorney or other referrer sends us a referral, we receive the patient's details, the claim details, the referrer's contact details and any medical records attached.
  • How forms reach us. Form submissions are delivered to our office by our website host, Netlify, which keeps a copy of each submission for our practice. Larger record files uploaded with a referral are held in our website's storage so our staff can download them, and are deleted automatically after 30 days.
  • Reading a referral sheet or answering by voice. If you choose to upload a photo of a referral sheet so the form fills itself in, the image is sent to our artificial-intelligence service provider, Anthropic, which reads the sheet and returns the fields. On a computer, if you choose to answer the referral questions by voice, your browser's own speech recognition turns your speech into text: Chrome and Edge send the audio to Google for this, and Safari uses Apple's. Some spoken answers are then sent to Anthropic to sort them into the right fields. Both features are optional; you can always type instead.
  • Returning referrers. We keep the names, phone numbers and email addresses of referrers, adjusters and case managers who have sent us referrals, so the form can fill in their details the next time they refer a patient. It does this only when someone enters a matching name together with a matching phone number or email address.

We use this information to schedule, evaluate and treat patients, to communicate with patients, referrers and claim professionals, and to keep our medical records. Information that becomes part of a patient's medical record is kept as the law requires for medical records.

Information collected automatically

  • Google Analytics. Most pages of this website use Google Analytics to show us how visitors use the site: which pages are viewed, how visitors arrived, and general information such as device type, browser and approximate location. Google Analytics stores cookies in your browser (named _ga and _ga_XLQB5BPMK6) to recognize repeat visits. It does not record what you type into our forms. Google may collect information about your activity across different websites over time; see how Google uses information from sites that use its services.
  • Website host. Like all websites, our host, Netlify, receives technical information with each visit, such as your IP address, browser type and the pages requested, to deliver and secure the site.
  • Fonts and videos. The site loads its fonts from Google Fonts, so Google receives your IP address when a page loads. The videos on our "Your Journey" pages are played through YouTube in its privacy-enhanced mode. When a video comes into view, YouTube, which is owned by Google, receives your request and may store information in your browser under Google's own privacy policy.

Information kept in your browser

Apart from the Google Analytics cookies, the site stores a few things only on your own device, for your convenience:

  • The referral form remembers the referrer's own contact details (not any patient's) so you don't have to retype them. Use "Not you? Clear" on the form to remove them.
  • After you send a referral, the confirmation page can show a one-time copy of the referral PDF. That copy is held briefly in your browser and deleted once it is shown.
  • If you use the voice option, the site remembers whether you want questions read aloud.

How we share information

  • With the service providers named above (Netlify, Google and Anthropic), only to run this website and its features.
  • With others involved in a patient's care or claim, such as insurers, workers' compensation adjusters and case managers, as permitted by law and described in our Notice of Privacy Practices.
  • When required by law.

We do not sell personal information, and we do not use it to show you advertising.

Your choices

  • You can block or delete cookies in your browser settings, or install Google's Google Analytics opt-out browser add-on. The site works without them.
  • Our website does not currently change how it works in response to "Do Not Track" signals from browsers.
  • To ask about, correct or request a copy of information we hold about you, call or write to us using the details below.

Security

The website uses encrypted (HTTPS) connections, and links to download uploaded records are private and expire. No method of sending or storing information online is completely secure, so if you'd rather not send information online, you can call our office instead.

Children

This website is not directed to children under 13. Forms for a minor patient should be completed by a parent or guardian.

Changes to this policy

We may update this policy as our website changes. The date at the top shows when it was last updated.
